Alzheimer’s Disease Respite Program
The Alzheimer’s Disease Respite Program provide in-home respite services to individuals with Alzheimer’s Disease or a related disorder as determined by a physician. Specially trained professional respite caregivers work in the homes of clients, providing relief for the primary caregiver.
Consumer Directed Options (CDO)
A program for Medicaid Waiver eligible consumers of any age who choose to direct their own care. Allows more flexibility in who provides services and when services are provided.
Kentucky Homecare Program
The Kentucky Homecare Program includes an assessment and case management component which is administered by the Purchase AAA. The services under the Homecare Program are provided to persons aged 60 or older who are functionally impaired.
Homecare services are meant to build on identified strengths, to compensate for weaknesses, and to acknowledge the client’s uniqueness. These services do not replace medical care, but are designed to help meet the needs of daily living. Services are available in all eight counties of the Purchase. Those services are:
Assessment: The collection of in-depth information about a client’s situation and function so a plan of care can be developed to meet his/or her needs.
Case Management: Ongoing client contact to ensure that appropriate, comprehensive, and timely services are received.
Homemaker/Home Management: Housekeeping services to aid a person in activities such as cleaning, shopping, budgeting, meal preparation and laundry.
Homemaker/Personal Care: Services to aid the client in personal care activities such as bathing, feeding, hair care, helping with toileting and dressing.
Respite Care: Limited in-home services to provide relief for the caregiver.
Home Delivered Meals: Nutritious lunchtime meal provided Monday through Friday to persons who are homebound due to an illness or disability.
Home Repair: Minor home adaptations and modifications to aid the elderly in maintaining independent living and to ensure health, safety or facilitate mobility.
Chore: Major cleaning assistance such as carpet cleaning, washing walls and removing debris.
Escort: Personal accompaniment of a physically, mentally, or developmentally disabled person who requires assistance to and from a medical appointment.
Personal Care Attendant Program
The Personal Care Attendant Program is a consumer-centered program for severely physically disabled adults (age 18 and older) who are at risk of institutionalization. Qualified consumers must be disabled in two or more limbs yet remain mentally capable of employing and supervising the attendants they employ.
The program provides qualified individuals with a subsidy to assist in securing personal attendants who provide the necessary services to enable the consumer to remain as independent as possible. The program is available in all eight counties of the Purchase Area.
