Here you can find forms used during the application for PADD's loan or loan guarantee programs and for the energy programs. Furthermore, we provide you with additional materials (flyer's, brochures, fact sheets, newsletters, etc.) that could be useful for you:

Not all forms and/or materials (or its contents) provided have been designed and/or developed by the Purchase Area Development District. Therefore, the Purchase Area Development District as well as all technical or informational sources referred to do not:

The Business Lending & Development Services Department is making every effort to provide you forms that can be used easily. Some forms can be filled out on your computer directly, others require a printout and a manual completion. Not all forms are designed by the PADD; some forms (i.e. starting with AD / IRS / RD / SBA / SF) are forms from governmental agencies that are necessary for the application process. The PADD has no influence on the design and complexity of these forms. All forms that are designed by the PADD have to fulfill certain state and federal requirements.

Even though the PADD is attempting to keep all forms current and updated, please be aware that some forms provided here could be outdated and therefore inadmissible. Please contact your Business Lending & Development Services Department for more information or if you suspect a form to be outdated.
